What To Expect

    We DO NOT participate in any vision plans, such as VSP or EyeMed.  Your examination and testing will be billed to your medical insurance. 

    Unfortunately, most medical insurances do not cover the cost of a refraction, so this fee will be a patient responsibility that will be collected at the time of service. Or you are free to proceed only with the medical portion of your exam here with us and to contact your insurance company to find an in-network provider for the refraction (glasses prescription) portion.

    We also do not sell glasses and you are welcome to take your eyeglass or contact prescription to be filled at any optical dispensary of your choosing.  If you have a vision plan, you can often use your vision plan toward the cost of glasses at an optical dispensary that participates with your plan. Please contact your insurance plan directly to find a participating optical shop.

  • Appointment times can vary. It is our goal to make sure you receive the time and attention you deserve, and that can differ for each patient. Please expect your visit to last between 60 to 90 minutes depending on your needs.  For your first appointment with us, please arrive 15 minutes early to allow ample time for check-in and paperwork. For established patients, please arrive 5-10 minutes early to review insurance information and contact information.

  • A dilated eye exam is recommended for all individuals and is necessary for Dr. Szymarek to perform a thorough examination. Expect to be dilated during your visit unless Dr. Szymarek instructs you otherwise.

  • Please bring your insurance card and government issued picture I.D. Most HMO and HIP Plans require a referral from your primary care doctor for a specialist visit. Please check with your insurance company if you are unclear as to your insurance companies requirements. If your plan requires a referral, please have the referral forwarded to us in order to be scheduled and for insurance to cover your visit.

    Credit Card on File Policy

    We are committed to providing you with exceptional care, as well as making our insurance billing processes as simple and efficient as possible. Given the ever changing environment in healthcare, more responsibility of payment is being placed on the patient in the form of copays and deductibles. Thus, it has become necessary to ensure we have a guarantee of payment on file in our office.

    Patients whose insurance plans have significant deductibles or coinsurance, or whose estimated patient responsibility exceeds $50 are required to maintain a credit card on file for balances after insurance processing.

    Payment is still due at the time of service. 

    We will continue to collect estimated Out of Pocket costs, such as copays, co-insurance amounts and applicable deductible amounts, at the time of your visit and send the claim to your insurance company. Upon the receipt of their determination of benefits, we will only charge your credit card when they inform us of additional patient responsibility. Circumstances when your card would be charged include, but are not limited to:

    • missed co-payments, deductible and co-insurance

    • missed or cancelled appointments without 24-hour notice

    • any non-covered services and / or denial of services allocated to patient responsibility, such as the fees associated with performing a refraction (determining your glasses prescription)

    • any amount not paid by your insurance 60 days after a corrected claim has been filed

    This in no way will compromise your ability to dispute a charge or question your insurance company’s determination of payment.

    Please come to your visit prepared with a valid form of payment.  Your credit card information will be kept safe and secure. No credit card numbers, other than the card’s last four numbers, will be visible to any staff at any time.
